Portal Login Credentials

Are NOT required to complete the following tasks in the portal:

  • The initial application submission from a new Facility.
  • The initial application submission from an existing Facility.
  • The request for a Hearing.
  • The request for Administrator/DON change.
  • The request for Portal help.

Are required to complete the following tasks in the portal:

  • The update and resubmission of any Facility Application.
  • The appeal of a rejected Facility Application.
  • The submission of a Plan of Correction.
